WordPress Form Templates by Industry: 16 Starter Forms and What Each One Asks

Most WordPress form templates are the same contact form with a different heading. An industry form is different when its 3 middle questions are the ones that trade actually asks first, and that is what this page is about. So, if you are looking for a WordPress form template for a clinic, a garage, a caterer, a law firm or one of the other 12 trades below, this guide can surely help you pick one and know what it will and will not do.

Sixteen trades cannot fit in one tutorial, so this page does a different job. It gives each form its fields, its button and what happens after submit, and it points to the full guide where one exists. The 5 forms with a guide of their own are marked, and the other 11 follow the same pattern closely enough that one of those guides will carry you through.
There is no download here, because the templates are already in the plugin under Core Forms > Add New. Without further ado, let’s get started.
What Every Industry Form Shares
Before the list, the part that is the same across all 16, so it is said once.
Every industry form has 3 steps. “Your details” holds name, email and an optional phone. “Your request” holds the 3 or 4 questions specific to the trade plus an optional “Anything else we should know?” box. “Confirm” holds one required checkbox, “You may contact me about this request,” and the submit button. The layout can be collapsed to a single page in the builder with one setting, and the step assignments are kept for switching back.
Every one arrives with the same settings and actions:
- Send Email to the site admin, with the subject “[Template name]: [NAME]” and a Reply-To header set to the sender.
- Auto-Responder to the sender, subject “[Template name] - Request received,” repeating the success message they saw on screen.
- Save submissions on, Prevent duplicates on, a rate limit of 10 per IP per hour and the honeypot on.
- Payment off, save and resume off, and no standalone public page.
And every one records a request. None of them reserves a slot, charges money, subscribes anyone to a mailing list or asks for a file. Consent to be contacted about the request is the only consent on the form, and it is kept separate from marketing on purpose.
That last paragraph is the reason to use these over a generic contact form. Each template already has the honest wording for its trade, such as “This is not a confirmed booking” on the accommodation form, and you are less likely to promise something a form cannot deliver.
Property Viewing Request
For estate agents and letting agents. The request step asks for Property interest (Buy, Rent or Commercial), a Preferred area and an optional Budget range. The button reads “Request a viewing,” and the success message says the agent will contact them to confirm availability.
The budget field is optional here, which is the right call. People will name an area to a stranger and a budget only to someone they have spoken to. The property viewing request template has the live demo.
Home Service Estimate
For plumbers, electricians, cleaners and repair trades. The request step asks for the Service needed (Plumbing, Electrical, Cleaning, Repairs or Other), the Service postcode and When do you need help? (As soon as possible, This week, This month or Flexible). The button reads “Request an estimate.”
The urgency select is what earns this form its place, because “as soon as possible” and “flexible” should not sit in the same queue. A recipient route on that field sends the urgent ones to whoever is on the van today. Construction Project Inquiry
For builders, contractors and fit-out firms. The request step asks for the Project type (New build, Renovation, Extension or Commercial fit-out), the Project location and the Current stage (Exploring options, Design ready, Permits in progress or Ready to start). The button reads “Send project inquiry.”

Agency Project Brief
For design, marketing and development agencies. The request step asks for the Business or organization, the Project focus (Website, Branding, Marketing, Content or Other) and the Preferred timeline (Within a month, Within three months or Flexible). The button reads “Send project brief.”

Course Enrollment Inquiry
For schools, training providers and course creators. The request step asks for the Course or subject of interest, the Preferred study mode (Online, In person or Either) and the Preferred start (Next available intake, Later this year or Still deciding). The button reads “Ask about enrollment.”

Clinic Appointment Request
For clinics, dental practices and physiotherapists. The request step asks for the Appointment category (New patient, Follow-up or General inquiry), a Preferred date and the Best time to contact you (Morning, Afternoon or Evening). The button reads “Request an appointment.”
This is the one template on the list built around what it does not ask. There is no symptoms field, no upload and no date of birth, because a form that collects health information needs hosting and handling a general website form does not provide. Legal Consultation Request
For law firms. The request step asks for the Area of inquiry (Business, Property, Employment, Family or Other), the City or region and the Preferred reply (Email or Phone). The button reads “Request a consultation.”
The area select is deliberately broad. A prospective client should not have to describe their matter on a public website, and a firm should not want them to before a conflict check. The legal consultation request template has the live demo.
Accommodation Inquiry
For guesthouses, B&Bs, holiday lets and small hotels that confirm by hand. The request step asks for the Preferred arrival date, the Number of nights and the Number of guests. The button reads “Check availability,” and the success message says plainly that this is not a confirmed booking.

Catering Inquiry
For caterers, restaurants that do events and venues. The request step asks for the Event date, the Expected guests and the Service style (Drop-off, Buffet, Seated meal or Not decided). The button reads “Request a catering proposal.”

Vehicle Service Request
For garages, tire shops and dealership service desks. The request step asks for the Vehicle make and model, the Service required (Routine service, Inspection, Repair estimate or Other) and a Preferred service date. The button reads “Request a service slot.”

Travel Planning Inquiry
For travel agents and tour operators. The request step asks for the Destination or region, the Number of travelers and the Trip style (Leisure, Family, Adventure, Business or Other). The button reads “Request travel planning.”
There is no date field, on purpose. A planning inquiry often starts before dates exist, and the message box takes “sometime in spring” better than a date picker does. The travel planning inquiry template has the live demo.
Fitness Membership Inquiry
For gyms, studios and personal trainers. The request step asks what the person is Interested in (Gym membership, Group classes, Personal training or Other), their Training experience (Getting started, Returning to training or Training regularly) and a Preferred time (Morning, Afternoon, Evening or Flexible). The button reads “Request membership information.”

Salon Appointment Request
For hair, nail and skin care salons. The request step asks for the Service of interest (Hair, Nails, Skin care or Other), a Preferred date and a Preferred time (Morning, Afternoon, Evening or Flexible). The button reads “Request an appointment.”

Freight Quote Request
For hauliers, couriers and freight forwarders. The request step asks for the Pickup city and country, the Delivery city and country and the Cargo type (Parcels, Pallets, Furniture or Other). The button reads “Request a freight quote.”
Weight and dimensions are left to the message box, because the first inquiry is about whether you cover the route at all. The freight quote request template has the live demo.
Volunteer Application
For charities and community groups. The request step asks for the Area of interest (Events, Community outreach, Administration, Fundraising or Other), Availability (Weekdays, Weekends, Occasional events or Flexible) and the City or area. The button reads “Apply to volunteer.”

Software Onboarding Request
For SaaS products with a human onboarding step. The request step asks for the Company or organization, the Team size (Just me, 2 to 10, 11 to 50 or More than 50) and the Primary goal (Initial setup, Migration, Team training or Integration help). The button reads “Request onboarding.”

Which One to Start From
Pick the template for your trade if it is on the list, even if you plan to change half the fields, because the wording of the button, the success message and the acknowledgement email is already right for that kind of request.
If your trade is not on the list, pick the nearest shape rather than the nearest industry. A photographer’s inquiry form is closer to the catering form (a date, a headcount, a style) than to anything with “creative” in the name, and a tutoring service is a course enrollment inquiry with the labels changed.

The Limits
None of these forms checks a calendar, so two people can ask for the same slot and both requests arrive. The confirmation is yours to send.
None of them takes a file. That is deliberate for a first inquiry, and a file field is a 1-minute addition in the builder when you need one.
The selects use fixed options. “Plumbing, Electrical, Cleaning, Repairs, Other” fits most trades and not all of them, so expect to rename options before you publish.
The message box is free text with a 2,000 character limit, and people will put in it whatever they want to say, including details you asked them not to include.

Can an industry form be a single page instead of 3 steps?
Yes. In the builder, change Form layout from Multi-step to Single page and save. The step assignments are kept, so you can switch back.
Do these forms take payments or deposits?
No. Payment is switched off on every industry template. The payment templates in the same gallery are the starting point for a form that charges.
Do these forms add people to a mailing list?
No. The consent checkbox covers contact about the request only. A newsletter needs its own opt-in field and an email marketing action.
