Core Forms creates a Zendesk ticket for each submission, with the person who filled in the form as the requester. Subject, description, priority, tags and custom ticket fields all come from the form.
Files uploaded through the form can travel with the ticket. Zendesk keeps control of its own notification emails, so your existing triggers and auto-responders keep working the way they do now.

Set low, normal, high or urgent, or map priority from a field. Tags accept fixed values or field variables.
Map numeric Zendesk field IDs to form values in a small JSON object.
Attach files from the form's upload fields. Limits are 10 MB per file, 20 MB in total and 20 files, and submissions must be saved.
Each ticket carries a reference tied to the submission, so a retry finds the ticket Zendesk already accepted instead of opening another.
In Zendesk Admin Center, open the API settings, enable token access and create a token.
Form → Actions → Add Action → Create Zendesk Ticket. Enter your subdomain, an agent email and the token.
Map requester email and name, set subject, description and priority, add tags or custom fields, then run Test action.
